You'll enjoy the flexibility to telecommute* from anywhere within the U.S. as you take on tome tough challenges. Senior Wellness Coordinators provide telephonic aftercare follow-up for enrollees once discharged from an inpatient treatment setting. Cases for follow-up are generated from inpatient authorizations entered into UBH's clinical documentation system.


Primary Responsibilities: 
  • Establish a working relationship with Participants using telehealth modalities to effectively provide care, coordination and advocacy

  • Provide preventative health screenings
  • Interview Participants regarding health behaviors, health outcomes and patient satisfaction

  • Work collaboratively with Optum Alaska Care Advocates and Medicaid Providers to support Participants as they transition between levels of care (typically from residential or inpatient to lower levels of care)

  • Work collaboratively with Care Advocates and Participants to regularly review treatment plans (Plans of Care) and update the plans as appropriate

  • Provide education and support that will help Participants meet their treatment plan (Plan of Care) goals

  • Work with Participants to identify and access resources, traditional and non-traditional, that will support their recovery and mental health rehabilitation

  • Partner with Peer Support Specialists to help Participants understand mental health and substance use diagnoses, access healthcare and community resources

  • Educate and refer Participants to local Mental Health First Aid trainings and Question, Persuade and Refer (QPR) training on laww.com

  •  Participate in Quality Assurance and Improvement (QAPI) activities; as directed by supervisor

  • May participate in cross-functional teams, projects and initiatives, process improvement activities, and typically requires previous health insurance experience
