The SeniorPatient Care Coordinator- responsible for assigning health plan members to engage them in Optums clinical programs.

""

This position is full-time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of (8:00am - 5:00pm). It may be necessary, given the business need, to work occasional overtime.

Training will be conducted virtually from your home.

Primary Responsibilities:

    ""
  • Demonstrates a high level of customer service behavior at all times
  • ""
  • Participates in quality monitoring process,
  • ""
  • Daily admission and discharge review and communicate with clinical team
  • ""
  • Monitor risk level change queues in EMR
  • ""
  • Assign members to clinicians
  • ""
  • Answer in bound member calls
  • ""
  • Monitor market line mailbox
  • ""
  • Expedites emergency calls as needed
  • ""
  • Attends regular staff meetings as scheduled
  • ""
  • Maintains confidential health information according to state and federal regulations including HIPAA.
  • ""
  • Responsible to assist with gathering and organizing of appropriate medical records (e.g. history and physical, lab work, test results).
  • ""
  • Performs other duties as assigned by supervisor.

  • 3+ years previous experience in healthcare customer service

Colorado Residents Only: The hourly range for Colorado residents is $17.12 to $30.34.
