POSITION DESCRIPTION

Position Title: Charge Nurse

Department: Nursing Services

Reports To: CNO, Nursing House Supervisor, Director, Nursing/Clinical Services

Supervises: Unit Nursing Personnel assigned to shift (LVN, PT, MHW, CNA, and Medical Assistants)

POSITION SUMMARY

Responsible for delivery of quality general and psychiatric nursing care to children, adolescents, adults, and geriatrics with emotional or psychiatric disorders; serve actively and constructively as a member of the multi-disciplinary treatment team, supporting the organization and program philosophy of care; to ensure the safety and well-being of each patient.

QUALIFICATIONS

Possesses current RNs license issued by the State of California; demonstrates sound leadership skills and utilizes these skills in organizing the activities and schedules for medical and/or non-medical tasks of the patient for a given shift; record of consistently high quality of clinical and interpersonal skills to be an exemplary role model to others; displays basic knowledge of treatment procedures; interventions common to acute psychotic as well as to non-violent crises intervention practice; basic knowledge of abnormal psychology as typically found in the psychiatric patient population; application of this knowledge to nursing care; basic knowledge of medical terminology especially those terms associated with psychiatric care; thorough familiarity of psycho pharmacy and the use of psychotropic medications; basic teaching and training skills helpful; problem-solving; organizational and time management; crisis intervention skills; written and oral communication skills; skills in facilitating and/or co-facilitating process orientated and didactic groups; must have current CPR certification (American Heart Association only) and eligible for PMAB certification; skills in establishing and maintaining effective working relationships in a variety of clinical and support team members; empathy for patients and their families; flexibility regarding hours and schedule worked; skills in maintaining information as highly confidential.

RESPONSIBILITIES/STANDARDS OF PERFORMANCE

General Responsibilities

  1. Adheres to hospital policies and procedures regarding absenteeism and attendance, clocks in and out as scheduled, and follows time clock procedures.
  2. Follows the hospital meal and rest break policy and ensures any direct reports are allowed time for meal and rest breaks (if applicable).
  3. Maintains and follows hospital/department dress code policy and represents the hospital in a professional manner at all times.
  4. Is willing to assume additional responsibilities as assigned.
  5. Maintains confidentiality of former and current patient information in accordance with HIPAA standards.
  6. Respects and safeguards the confidentiality and privacy of Hospital and co-worker information.
  7. Understands and complies with the intent of the Compliance Program.
  8. Maintains proper boundaries when interacting with patients and their families, visitors, professional staff, vendors, and co-workers. Seeks clarification from supervisor or Human Resources when necessary.
  9. Adheres to and supports the Employee Drug Screening Policy.
  10. Demonstrates fiscal responsibility by effectively utilizing resources in a cost efficient manner.
  11. Conduct on and off site reflects favorably on Canyon Ridge Hospital.
  12. Has the knowledge of growth and development for patients of all ages. Demonstrates an understanding of the psychological needs of these groups and basic interventions used for different age groups as evidenced by the attached Age Specific Competency Checklist.

    Performance Improvement/Education
  26. Participates as assigned in the hospital and departments Performance Improvement Program.
  27. Completes competency tests, programs, and self-assessments as required.
  28. Attends all mandatory training as required and assumes personal responsibility for his/her annual education.
  29. Maintains and forwards up-to-date documentation of certifications, registrations and licenses to Human Resources for placement in employment records.
  30. Interactive during training session completing post test, training evaluations and other related documents.
  31. Receptive and non-defensive toward feedback and training recommendations.
  32. Contributes to departmental orientation of hospital/department personnel.

Safety Rules and Procedures/Infection Control

  1. Complies with safety and security policies and procedures, including those regarding incidents and injuries.
  2. Keeps workspace clean and free of work hazards and follows hospital hazard communication programs.
  3. Complies with hospital policy regarding identification badge.
  4. Maintains equipment in safe working order, reports problems to supervisor and/or maintenance
  5. Responds to codes in accordance with policy and procedures.
  6. Practices Standard precautions. Recognizes and reports potential infections, completes PICS in a timely manner, utilizes effective patient teaching to prevent spread of infection.
  7. Demonstrates working knowledge of nursing process and all legislation that affects licensure and patient care. Relates nursing theory and principles to clinical practice.
  8. Is knowledgeable of hospital and nursing policies and procedures. Takes initiative to locate and clarify when unsure of specific policy.
  9. Coordinates clinical function of Unit in collaboration with Director of Nursing and other hospital departments. Understands scope of position and its relationship to other hospital departments.
  10. Determines staffing needs on Unit (as needed in absence of Staffing Coordinator). Adds or subtracts staff according to census and acuity. Coordinates with other Units to ensure facility-wide staffing needs are met.
  11. Makes daily patient care assignments of staff members based on acuity and level of competency of staff. Properly instructs and guides MHWs and LVN/PTs in patient care responsibilities and assignments.
  12. Encourages maximum utilization of skills, demonstrates effective and consistent leadership through teaching, assisting staff and providing direct patient care.
  13. Assists Director of Nurses with performance evaluations of employees assigned to unit and continuously monitors and coaches staff performance.
  14. Recognizes unsatisfactory nursing care performance, coaches and/or counsels employees as necessary, documents action and outcome when counseling is warranted and continuously monitors for improvement.
  15. Makes effective recommendations for department training activities and methods.
  16. Demonstrates knowledge of skin care treatment and breakdown, and effectively performs ordered skin treatment.
  17. Treatment Planning Skills; develops a comprehensive outline of care to be delivered to attain expected outcomes; included in this plan, specific goals and interventions delineating nursing actions unique to each patients needs.
    • Assesses and documents problems to be identified on Master Treatment Plan.
    • Effectively develops measurable long and short-term treatment goals.
    • Outlines Nursing interventions to meet goals which specifically identify who, what, where, when and why for each intervention.
    • Treatment plan and progress notes clearly reflect progress of patient and family toward goals and response to interventions.
    • Establishes effective discharge plans, individualized and relevant to needs of patient and significant others.
    • Plans care for identified active medical problems on the Master Treatment Plan.
    • Resolves/modifies Treatment Goals as needed.
    • Document findings of response to treatment on RN Assessment Form and in Progress Notes.
  18. Continuously monitors level of competency in the area of medical record documentation by the staff assigned to Unit. Provides ongoing education in the area of accurate documentation. Communicates with Clinical Coordinator/DON. Provides updates on patient care, treatment, and emergency situations.
  19. Utilizes Nursing skills to accurately perform ongoing assessment of patients psychiatric and medical condition.
  20. Utilizes therapeutic interventions (group skills, crisis, and milieu) in interactions with patients and their families.
  21. Utilizes therapeutic group techniques to effectively lead Nursing groups. Utilizes crisis intervention techniques to assess and safely manage patients/families in crisis, and documents response to interventions. Utilizes milieu management techniques to ensure the safe and therapeutic management of the Unit milieu. Utilizes reality orientation, limit setting and redirection techniques effectively and documents patient response to interventions.
  22. Patient/Family Teaching Needs:
  • Accurately assesses the knowledge deficits of patients and families.
  • Utilizes effective teaching techniques with patients/families.
  • Documents patient/family teaching performed on the Multi-Disciplinary Teaching Flow Sheet.
  1. Nursing Multi-Disciplinary Assessment; through a systematic process of interacting with patient, the patients significant others, and the appropriate health care providers, collects and analyzes data regarding the patients physical, psychosocial, environmental, self-care, educational, and anticipated discharge planning needs.
  2. Reviews indicate that special precautions have been implemented as necessary and in accordance with established protocols and physicians supervision and direction has been properly sought.
  3. Room assignments are appropriate to patients needs, age, gender, level of functioning and condition; unit placement recommendation reflects patients ability to interact within the therapeutic community.
  4. Assists M.D. in special procedures, H&P, medical procedures.
  5. Transcribes and notes Physicians orders accurately per policy.
  6. Ensures that physicians are notified of any problems associated with their patients, i.e., abnormal lab, suicidality, special request, acting out behavior and documents these appropriately in the patient medical record and on the Incident Report Form
  7. Medication Administration/Management;
  • Demonstrates knowledge of use, effect, side effect of antidepressant, anti-anxiety, anti-mania, and antipsychotic medication, and medications prescribed for medical problems.
  • Safely and accurately administers medications as ordered in accordance with hospital policy and procedure.
  • Provides teaching for patients and families on the therapeutic, side effects and untoward effects of medication and other substances.
  • Documents medications administered, records any incidence of side or untoward effects and actions taken.
  1. Ensures that regular and accurate patient rounds are assigned, reports untoward patient conditions to DON as needed.
  2. Responds as back-up to Assessment & Referral, completes Intake Form accurately and thoroughly, and informs Intake Department of call in a timely fashion. Upon request, obtains 5150 certification from the County of San Bernardino.
  3. Understands and utilizes CPI techniques:
    • Recognizing cues to behaviors.
    • Using less restrictive interventions.
    • Patients Rights issues.
    • Appropriate use of time out.
    • Use of seclusion.
    • Use of restraint.
    • Documentation

PHYSICAL, MENTAL, AND SPECIAL DEMANDS

Reaching: Extending hand(s) and arm(s) in any direction.

Walking: Moving about on foot to accomplish tasks, particularly for long distances.

Fingering: Picking, pinching, typing or otherwise working, primarily with fingers rather than with the whole hand or arm as in handling.

Grasping: Applying pressure to an object with the fingers and palm.

Talking: Expressing or exchanging ideas by means of the spoken word. Those activities in which they must convey detailed or important spoken instructions to other workers accurately, loudly, or quickly.

Hearing: Perceiving the nature of sounds with no less than a 40db loss @ Hz, 1,000 Hz and 2,000 Hz with or without correction. Ability to receive detailed information through oral communication, and to make fin discriminations in sound, such as when making fine adjustments on machined parts.

Repetitive motions: Substantial movements (motions) of the wrist, hands, and/or fingers.

Non-Sedentary Work: Exerting up to 100 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. May be exposed to second hand smoke as a result of patients smoking.

Ability to exercise self-control in potentially volatile situations such as being verbally or physically confronted in a threatening or aggressive manner; must be able to work and concentrate amidst distractions such as noise, conversation and foot traffic; ability to handle interruptions often and be able to move from one task to another; must be flexible and not easily frustrated in dealing with differences of opinions.

MACHINES, TOOLS, EQUIPMENT, AND OTHER WORK AIDES:

Telephone, TV/VCR, copy machine, shredder, shower/bath, thermometer, blood pressure cuff, Glucose Monitoring System, oxygen tank, physical restraints, calculator, refrigerator, first aid items, CPR face shields/mouth guards.
