About the Role

""

The Registered Nurse is responsible for improving their patients overall type 2 diabetes management and well-being. The nurse will serve as a telemedicine nurse and case manager across multiple continuums of care, often needing to independently identify solutions through critical thinking skills. They will act as an advocate for patients, collaborate with physicians, advanced practitioners, clinical staff and community resources utilizing a proactive, multidisciplinary team-based approach directed toward prevention, education, and health promotion.

Responsibilities

    ""
  • An effective team member who contributes valuable ideas and feedback and can be counted on to meet commitments.

  • Assess, screen, triage, and perform nursing telemedicine visits and treatments ordered for patients.
  • ""
  • Direct telemedicine patient care, case management, and follow-up.
  • ""
  • Complete medication reconciliations and update medical history to prepare Provider reviews.
  • ""
  • Evaluate patients for medication de-escalation triggers.
  • ""
  • Identifies emergency situations and provides care within legal scope of practice.
  • ""
  • Participate in outreach/follow-up phone calls and follow-up patient visits.
  • ""
  • Assist in the development of criteria to measure outcomes, assists in the analysis of data for quality improvement, interpret study results, identify limitations and barriers and recommend alternatives.
  • ""
  • Develop and present educational services for patients or departments as appropriate.
  • ""
  • Support a proactive, multidisciplinary team approach directed toward prevention, education and health promotion.
  • ""
  • Establish supportive and trusting relationships with the patient and acts as a resource.
  • ""
  • Assess patients educational/clinical needs, prioritizing and individualizing program based on elicited information.
  • ""
  • Provide disease specific self-care education.
  • ""
  • Collaborate with Physicians or other Licensed Independent Practitioners and other nurses to coordinate further medical treatment when needed.
  • ""
  • Consult with other members of the healthcare team to solve problems and seek advice. Design interventions to improve clinical outcomes.
  • ""
  • Apply the use of logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, and approaches to problems.

  • Prioritize and multitask to optimize patient experience.
  • ""
  • Ensure the well-being of patients and provide a positive, supportive environment for patients and other staff.

  • Complete EMR documentation and utilize documentation systems for all assessments and procedures.

  • Comply with and follow policies and procedures as outlined by Level2.

  • Maintain the perseverance to drive and sustain the changes that occur at Level2 while being resilient and flexible and inspiring and motivating the team.

  • Participate in quality improvement projects to include the following topics: patient/primary care provider relationship, accessibility to care, comprehensiveness of care, continuity and/or coordination of care and clinical study.

Required Qualifications

    ""
  • Graduate of an accredited School of Nursing with current state licensure
  • ""
  • 4+ years of clinical experience
  • ""
  • Required to maintain CMEs as applicable for licensure
  • ""
  • Proficient with computerized information systems (EMR) used in clinical management settings
  • ""
  • Perform duties in accordance with the customary rules of ethics and conduct of the applicable State Board of Nursing and other such regulating bodies
  • ""
  • Able to meet the needs of the patient by processing requests and referrals, providing efficient follow-up with the patient, pharmacy or other entity, as needed
  • ""
  • Strong commitment to Customer Service and Excellence in healthcare
  • ""
  • Ability to communicate effectively both orally and in writing
  • ""
  • Proficient in Microsoft Word, Microsoft Excel, and electronic communication software programs
