JOB SUMMARY

Responsible for receiving, recording, disseminating, and tracking of referrals from markets assigned to the KidLink Network. Receives information from referral entities as well as distributes information to KidLink supported facilities and Service Advocates. Maintains the referral database so that cases are appropriately tracked throughout referral process from receipt of referral to admission into network. Works jointly with Service Advocates in communicating with customers and facilities the status of youth referred for admission.

QUALIFICATIONS

  • Bachelors degree in Human Services field or prior experience working in a residential treatment environment.
  • Proficient in the use of electronic correspondence and word processing.
  • Prior experience working in marketing or referral development for a mental health or healthcare agency, preferred.

Environmental Conditions: The work environment for this position requires that the employee be able to maintain an office within a facility or at corporate office. The employee may visit or work out of a facility. While performing the duties in facilities the employee may be exposed to clients who may ask inappropriate personal questions, display socially unacceptable behavior, use profanity and sexually explicit phrases. Make insulting remarks or threats regarding appearance, age, sex, or race, exhibit defiance, dishonesty, and assaultive or self destructive behaviors. The employee is occasionally exposed to outside weather conditions, and may be subject to travel as required by supervisor.

Physical Requirements: Ability to communicate verbally, telephonically and in writing. Push, pull and lift up to 40 pounds. Intermittent sitting, standing, walking, bending and stooping. Extensive use of computer terminal. Drug and alcohol free as evidenced by negative results of post-offer, pre-employment urine drug screen. The physical requirements described here are representative of those that must be met by an employee to successfully perform the functions of this job. ESSENTIAL DUTIES AND RESPONSIBILITIES

1.

Responds immediately to service and referral inquiries.

2.

Provides referral process and program information to potential patients, their families and referral sources.

3.

Upon receipt of referral packets/information ensure packets are complete, enter referral in database, generate Referral Information Sheet (RIS) and send referral to appropriate network facilities within one working day of receipt. Send RIS to assigned Service Advocate.

4.

Track each referrals disposition to all network facilities receiving the referral, demonstrating every effort to secure a disposition from the facility within one working day of receipt. Upon receipt of the agency disposition verbally inform the referral source and/or custodian of the acceptance and send a letter of acceptance.

5.

Maintain notes of all contacts involving each referral as well as update status of referral in the referral database and generate reports of those contacts to service advocates as requested.

6.

Assist facilities in obtaining needed information to render a timely disposition

7.

Review flash / admission & discharge report when received from assigned facilities and update database accordingly.

8.

Exhibit for KTS at industry trade shows as needed.

9.

Facilitate the development of and maintain adequate stock of program information and/or be able to generate program information as needed.

10.

Uses written or electronic correspondence to communicate activity related to referrals being processed, admission case management, customer service issues and any other requested or sales related activity.

11.

Maintains and organizes current information of services on computer, and maintains an adequate supply of information to be mailed or otherwise distributed.

12.

Assists when necessary with developing new service information.

13.

Facilitate weekly conference calls with assigned facilities to review their referral status report to obtain updates on status of referrals as well as current census and scheduled admissions and discharges.

14.

Participate in weekly KidLink team conference call to provide updated information on all assigned cases.

15.

Utilize Medik and other systems to reconcile facility entered data with that of the call center.
