ProHealth Physicians is seeking a Physical Therapist to join our Middletown team. Outpatient experience is necessary. The right individual will be customer service oriented with strong communication and patient management skills. Candidates must be licensed in the State of Connecticut. New graduates are welcome to apply and take advantage of our mentorship program. We offer excellent CEUs benefits. ProHealth Physicians offers excellent benefits and an opportunity to provide 1:1 quality care with one of the states leading primary care organizations. This is a 40 hour/week position. Office visits are typically from 7:00 am 7:00 pm. The exact schedule will be determined upon hire by the Hiring Manager. May be asked to float to local practices for coverage.

Primary Responsibilities:

Implements, with minimal supervision, high quality physical therapy services for patients, caregivers and facilities in accordance with the principals and practices of physical therapy and with ProHealth Physicians policies and procedures. Diagnose and treat individuals who have physical conditions that limit their abilities to move and perform functional activities in their daily lives by using treatment techniques to promote the ability to move, reduce pain, restore function, and reduce disability.

  • Evaluates and treats musculoskeletal dysfunction, balance, vestibular problems and other medical conditions
  • ""
  • Develops, supervises and carries out treatment plans, providing quality care to patients
  • ""
  • Supervises, directs and shares patients with Physical Therapist Assistants
  • ""
  • Initiates communication and develops positive interaction with patients, caregivers, and referral sources
  • ""
  • Maintains and develops positive interaction with clinical staff including PTs, PTAs and referring M.D.s and APCs as well as administrative staff
  • ""
  • Understands and adheres to compliant billing and documenting procedures to operate within compliance with insurance and federal/state regulations
  • ""
  • Seeks appropriate resources when issues arise
  • ""
  • Responsible for individual productivity goals and facility metrics to align with company goals and objectives
  • ""
  • Participate in PHP sponsored and independent continuing education opportunities including lab-based courses, clinical projects, journal review, and peer case review

  • Participates in company sales and marketing events, including community outreach programs

Required Qualifications:

  • Graduate from an accredited PT program
  • ""
  • Unrestricted CT license
  • ""
  • Outpatient experience
  • ""
  • EMR experience
  • ""
  • You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role
  • ""
  • Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained.
