This is a per diem position. You will beexpected to be available at least 24 hours/month and 16 of those hours should be during off-hours (evenings/weekends).This position would be covering LTC facilities in theNewark NJ Area.

In this role, you will represent Optum as you provide primary care to patients in long term care settings; coordinate with their Primary Care Provider and facility staff to deliver high quality care on site. Primary Responsibilities

• Develop strong competencies in Palliative Care as you provide preventative and primary care for patients in assigned long term care facilities

• Collaborate with primary care physicians, the nursing staff and the patients' families to coordinate care in the manner best suited to the patients needs

Required Qualifications

• Current, unrestricted NP license in the state of assignment

• Ability to obtain DEA licensure / Prescriptive Authority post - hire in states where applicable

• Clinical background and national certification as ANP, FNP, GNPrequired

• Past experience working in a nursing home or with seniors in other settings
""
• Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook
