The Care Management Department has an outstanding opportunity for a temporary, day shift, NURSE CARE COORDINATOR. The goal of the Nurse Care Coordinator is to ensure delivery of optimal care for the patient while promoting fiscal responsibility. SCHEDULE: This is a day shift position at 8 hours per shift with variable days per week to cover for planned leaves, weekdays and weekends. RESPONSIBILITIES: REQUIREMENTS UW Medicine includes Harborview Medical Center, UW Medical Center - Montlake, UW Medical Center - Northwest, Valley Medical Center, UW Neighborhood Clinics, UW Physicians, UW School of Medicine, and Airlift Northwest.?? |
