If you are located in Tucson, AZ, you will have the flexibility to telecommute* as you take on some tough challenges.

Primary Responsibilities:

  • Positions in this function require LPN/LVN unrestricted nurse licensure and certification based on role and grade level
  • In this role you will start your day by logging on to various hospital databases and reviewing new admissions and discharges
  • You will identify the high-risk patients and review their charts and lab results to determine current status
  • You will proactively reach-out to the doctor and the patient, introduce yourself, advise of your role and that you will be assisting the patient to ensure they are following their treatment plan and have the resources they need
  • The majority of your time will be spent on the phone with patients, family members, and community resources
  • Must be willing and able to occasionally make home visits
  • Will be responsible for conducting weekly conference calls to update patient care teams of specific patient's progress
  • Responsible for overseeing and participating in day to day clinical care of patients
  • Employees are involved in direct patient care (in a clinic, in-home care, etc.)

Required Qualifications:

  • Undergraduate degree or equivalent experience

  • At least 2 years' experience doing case management and/or care coordination in primary care clinic setting

  • Requires current unrestricted LPN licensure in applicable state

  • Must have valid, unrestricted Drivers License

  • Must be organized, dependable, proactive, and assertive

  • Ability to exhibit genuine care and compassion during every patient interaction is essential to this role

  • You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role

  • Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained
