Southwest Medical part of Optumcareis a multi - specialty group of physicians, Nurse Practitioners, and Physician Assistants consisting of over 300 providers, 21 medical health centers including six urgent care clinics, four convenient care centers, two lifestyle centers catering to older adults, an outpatient surgery center, home health, hospice and palliative care services, plus E - visits and online options via Now Clinic telemedicine technology. Southwest Medical offers patients compassion, innovation, and quality care throughout southern Nevada. Southwest Medical is headquartered in the greater Las Vegas, NV area.

TheLPNwill support the Southwest Medical'sLifestyle Centers.They will be responsible to oversee Medical Assistants and assist as needed. They will be giving all injections, prepping charts, assisting with rooming patients,using mediclick for ordering supplies, responsible for tasks, doing QC's on rotating basis, checking all expiration dates in preparing for audits, performing mock codes, CAP testing and other assigned duties by the supervisor.

***This position is Monday - Friday with a focus on senior patient care***

Primary Responsibilities:

  • Responsible for collecting all the pre-visit / HEDIS / STAR information on over 800 patients that come to the LSC for their annual wellness SD exams each month
  • Evaluate ancillary walk in patients for the Lifestyle team, assist with all the injections and medications needed to be given to any patients during their visit for the team and will be required to complete approximately 300-500 support tasks each month
  • Provide excellent customer service and help identify opportunities with patients that allow us to adhere to, and improve, our quality measures/star measures for patient care
  • Assist with phone calls and messages
  • Determines best actions and collaborates with team to assign accountabilities to resolve
  • Responsible for collection and documentation of patient information to include vital signs, chief compliant, review of medications, allergies health risk screen information and other services
  • Ability to direct patients to the team member that would resolve issues prior to and after theLifestyle Center appointment
  • Functions as a member of the interdisciplinary team to facilitate care, assists RN with the coordination and completion of patient care plan
  • Responsible for compliance requirements such as laboratory QC management, waive testing, maintenance of crash cart and drug count, as well as other requirements dictated by clinic needs
  • Provides clinical intervention, such as drug administration, treatments, and procedures as ordered by provider and within scope of practice
  • Other duties as assigned to include, but not limited to, assisting coworkers, float to other areas and training of Medical Assistants
  • Follow standard protocols, processes, and policies to include but not limited to the following: Medication Refill, Preventative Services, Managing Conditions, and Disease Case Management as signed by department head or designee

Required Qualifications:

  • Unrestricted current LPN license in the state of Nevada
  • Proficient with Microsoft Office systems and Electronic Medical Records (EMR)
  • CPR certification or willing to obtain within 30 days of hire
  • Willing to float to other Southwest Medicalfacilities with providers as needed
  • 4 + years of clinical nursing experience required
  • Must have reliable transportation, proof of insurance and valid Nevada drivers license
  • You will be provisioned with"appropriate Personal Protective Equipment (PPE) and are required to perform"this role with patients and members on site, as this is an essential function"of this role.

  • Employees are required to"screen for symptoms using the ProtectWell mobile app, Interactive Voice"Response (i.e., entering your symptoms via phone system) or a similar"UnitedHealth Group-approved symptom screener prior to entering the work site"each day, in order to keep our work sites safe. Employees must comply with any"state and local masking orders. In addition, when in a UnitedHealth Group"building, employees are expected to wear a mask in areas where physical"distancing cannot be attained.
