The Lead Behavioral Health Clinician (Lead) will partner with the Primary Care at Home Leadership team to supervise the clinical activities of the National Behavioral Health Team and provide direct patient care within the Iowa Market of MyConnections. The Lead will drive strategies to reinforce the behavioral health principles that inform the MyConnections philosophy for the entire Clinical Redesign team. This role will be responsible for creating and maintaining the programmatic guidelines under which the Behavioral Health Team practices and engages with members and colleagues. The role will collaborate with the Primary Care at Home Leadership team and Chief Behavioral Health Officer to oversee all aspects of the Behavioral Health Team including: Patient Care, Programming, Quality, and Relationship/Culture.

The behavioral health team (including the Lead) provides integrated behavioral healthcare in collaboration with a primary care nurse practitioner and/or physician to chronically ill and high risk patients in a variety of environments including private homes, and may include assisted living facilities, nursing homes, skilled nursing facilities, and/or hospital settings.  The BHC exercises autonomy in behavioral health care decision making and trauma informed care principles while collaborating with a multidisciplinary team to engage resources and strategies to address medical, functional, and social barriers to care.


Clinical Redesign is a business unit focused on developing and deploying innovative direct care models for underserved populations.  The business unit is housed within Community& State, the Dual Special Needs insurance arm of UnitedHealthcare.  The Clinical Redesign Team is deploying a range of direct care models targeting both complex and healthy populations including members with medical, mental health, addiction, and social complexity with a trauma-informed approach.  As the R&D, innovation team within Community & State, the business unit is charged with transforming the way care is delivered, to achieve better care at lower cost. The team also draws upon a range of business best practices to ensure the services will scale and spread with strong fidelity and high quality service delivery.


This position is located in Des Moines, IA and is responsible for clinical care delivered in markets across the country.


Primary Responsibilities:

  • Management
    • Overall accountability for the Behavioral Health Clinician care delivery, including clinical oversight, case consultation, quality assurance, outcome measurement and reporting, review of best practice, and evaluation of competency
    • Function in a leadership role during national learning circles, case conferences, clinical office hours, and trainings
    • Stay abreast of current clinical advancements in the field and bring information forward as consideration for the broader team
    • Serves as a subject matter expert to leadership team
  • Clinical Care
    • Implementation of face-to-face trauma-informed practices and interventions
    • Curation of EBPs and assessment of the applicability and practicality for the population served (e.g. trauma-informed CBT, brief solution focused therapy, DBT, EMDR, etc.)
    • Assess level of functioning and appropriate care plan (treatment plan) goals and objectives
    • Assess risk levels and develop associated safety/wellness plans
    • Provide diagnostic impressions and collaborate with prescribers, as necessary, for pharmacological intervention to address symptomology
  • Programming
    • Collaborate across markets to create policies and procedures that support the work of the Behavioral Health Team
    • Collaborate across markets to design, and advise on the creation of templates and assessments in the EMR
    • Serve as subject matter expertise and provide trainings/opportunities to develop staff skill and understanding
    • Further deepen the Behavioral Health Integration into all aspects of programming
  • Quality
    • Collaborate across markets to develop quality measures and outcomes
    • Ensure that care delivery and workplace practices adhere to NASW Code of Ethics or other similar framework that has national recognition
    • Complete quality reviews
    • Ensure care is delivered using best practice and aligns with the CRT Clinical Philosophy
    • Develop a peer review process
  • Relationships/Culture
    • Build and maintain collaborative and high-trust relationships with internal and external partners
    • Create and maintain a non-hierarchal culture of accountability and integrity that is compassionate, patient-directed, and free of judgement
    • Foster an integrated approach to patient care
    • Make recommendations to support the development of a trauma-sensitive workplace environment
    • Provide individual and group support/education to colleagues that aligns the local clinical practice with the CRT Clinical Philosophy of Care
