This position is full-time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8 hour shift schedules during our normal business hours of 8:00am CST - 5:00pm CST. It may be necessary, given the business need, to work occasional overtime. We are located at the University Medical Center in New Orleans, LA and you will be working onsite once COVID restrictions are lifted.

The Hospital Care Transition Coordinator will access the reporting tool to obtain names of everyone who is in hospital and prioritize high risk members/patients to meet with. These members need the most help and typically have difficulty finding placement after being in the hospital (indigent, IV drug users, homeless, etc.). The Hospital Care Transition Coordinator will interact with patients at bedside, work closely with the case manager/discharge planner at facility, and assist in finding locations of lower level care for these members (rehab, etc.). This will include making phone calls to find a facility who will accept member. The goal is to transition the member out of hospital more quickly and make sure they have access to community resources to help keep them out of hospital. Once a member is discharged, the Hospital Care Transition Coordinator will close the member's case. The number of meetings are based on patient need but for this type of patient, the Hospital Care Transition Coordinator should expect ongoing/multiple meetings throughout the member's hospital stay. The HCT will be provided with a laptop and portable printer, however there is not a designated workspace at the hospital for the Hospital Care Transition Coordinator at this time. This position will be remote for a short while and will be returning to the office once COVID Restrictions are lifted

Primary Responsibilities:

  • Obtain discharge disposition / location, including contact information post discharge
  • ""
  • Provide INN facility list for Secondary Level of Care transfers - AIR, LTACH, SNF, Other
  • ""
  • Participate in coordinating hospital transfers
  • ""
  • Assist with coordination of home care services (Home health, Home infusion, DME, Pharmacy)
  • ""
  • Assist with coordination of community services (Transportation, Adult day care, Meals on Wheels, Other)
  • ""
  • Provide member benefit education (Prescriptions, Member concerns, Chart / Medical history concerns)
  • ""
  • Validate / Schedule follow - up appointment with PCP / specialist
  • ""
  • Ensure member has an assigned PCP
  • ""
  • Coordinate post - discharge program communication (CTP, CM/DM)
  • ""
  • Assist members with understanding of discharge, transfer plans, instructions
  • ""
  • Remove barrier(s) to discharge

Required Qualifications:

  • High School Diploma / GED (or higher)
  • ""
  • 3+ years of experience in a hospital, acute care, or direct care setting
  • ""
  • Current unrestricted licensed practical nurse (LPN) in the state of Louisiana
  • ""
  • Ability to type and navigate Windows / PC applications
  • ""
  • Ability to work Monday - Friday, 8:00 am - 5:00 pm CST
  • ""
  • Must reside within 60 miles of New Orleans, LA and able to work onsite at University Medical Center after COVID restrictions are lifted
  • ""
  • You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role
  • ""
  • You will be asked to perform this role in an office setting or other company location, however, may be required to work from home temporarily due to space limitations
  • ""
  • Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained
