Our onsite Health Concierge is critical to building relationships with senior residents, encouraging them to take action in their care, and serving as the first point of contact for their healthcare needs. In this field based role, you will travel day to day to multiple facilities in the Buffalo area and work with customers face to face.

This position is full-time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 6:00am 6:00pm. It may be necessary, given the business need, to work occasional overtime. Our office is located at 50 Lakefront BLVD, Buffalo, NY.

Primary Responsibilities:

  • Proactively connect with residents and facility personnel within main-living areas
  • ""
  • Meet with UHC residents in 1:1 setting to complete program specific assessments
  • ""
  • Provide support to patients based on identified needs
  • ""
  • Accurately document all face-to-face interactions and phone calls within 24 hours
  • ""
  • Serve as healthcare quarterback (identifying issues and remediating as appropriate with experts across the company)
  • ""
  • Always putting the customer/resident first
  • ""
  • Maintains schedule adherence and good attendance
  • ""
  • Maintains confidential health information in accordance with the law

Required Qualifications:

    ""
  • High School Diploma / GED (or higher)
  • ""
  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
  • ""
  • 1+ years of experience in customer service
  • ""
  • 1+ years of experience in health care, social work, or office administration
  • ""
  • 1+ years of professional experience in an office setting using the telephone and computer as the primary instruments to perform the job duties
  • ""
  • Ability to work any of our 8-hour shift schedules during our normal business hours of 6:00am 6:00pm. It may be necessary, given the business need, to work occasional overtime
  • ""
  • You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role
  • ""
  • If you need to enter a work site for any reason, you will berequiredto screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or similar UnitedHealth Group-approved symptom screener. When in a UnitedHealth Group building, employees are required to wear a mask in common areas. In addition, employees must comply with any state and local masking orders
