This position is full - time (40 hours / week) Monday - Friday. Employees are required to work our normal business hours of 8:00am 5:00pm. It may be necessary, given the business need, to work occasional overtime or weekends.

  • Create a positive experience and relationship with the member

  • Keep the member out of the hospital by supporting regular visits to their primary physician

  • Keep member actively engaged with their primary physician

  • Support the member to ensure pick - up of their Rx

  • Proactively engage the member to manage their care
  • ""
  • Provide member education
  • ""
  • Support transitions of care
  • ""
  • Help to keep members compliant with their care plans
  • ""
  • Partner with care team (community, providers, internal staff)
  • ""
  • Knowledge and continued learning of community cultures and values

  • May conduct member assessments if needed

Required Qualifications:

  • High School Diploma / GED (or higher)
  • ""
  • Resided within the local community for 2+ years
  • ""
  • Knowledge of culture and values in the local community
  • ""
  • Familiarity with the resources available within the community
  • ""
  • Field based experience (such as visiting members homes)
  • ""
  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
  • ""
  • Ability to create, copy, edit, send, and save documents, spreadsheets, correspondence Microsoft Word, Microsoft Excel, and Microsoft Outlook
  • ""
  • Ability to travel up to 75% of the time
  • ""
  • Residing inResiding in Westland, MI, Dearborn Heights, MI, MaComb, MI, Lapeer, MI, or Saint Clair, MI
  • ""
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high - speed internet service
  • ""
  • Ability to work our normal business hours of 8:00am 5:00pm, Monday - Friday
  • ""
  • You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role
  • ""
  • Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained.

  • Knowledge of the MI Medicaid and Medicare population
