As aField Based Community Health Worker (CHW),you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education.

Primary Responsibilities:

  • Create a positive experience and relationship with the member

  • Keep the member out of the hospital by supporting regular visits to their primary physician
  • ""
  • Keep member actively engaged with their primary physician
  • ""
  • Support the member to ensure pick - up of their Rx
  • ""
  • Proactively engage the member to manage their care
  • ""
  • Provide member education
  • ""
  • Support transitions of care
  • ""
  • Help to keep members compliant with their care plans
  • ""
  • Partner with care team (community, providers, internal staff)
  • ""
  • Knowledge and continued learning of community cultures and values
  • ""
  • May conduct member assessments if needed

Required Qualifications:

  • High School Diploma / GED (or higher)
  • ""
  • 2+ years of Care Management Experience
  • ""
  • Must have resided within the local community for 2+ years
  • ""
  • Knowledge of culture and values in the local community
  • ""
  • Familiarity with the resources available within the community
  • ""
  • Experience with outbound calls to members, Physicians, Pharmacy, Providers or similar medical facility
  • ""
  • Knowledge of disease management and treatment
  • ""
  • Ability to create, copy, edit, send & save utilizing Microsoft Word, Microsoft Excel, Microsoft Outlook
  • ""
  • Ability to navigate a PC to open applications, send emails, and conduct data entry
  • ""
  • Ability to be in the field 60% and home based 40%
  • ""
  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
  • ""
  • Must live in Phoenix, AZ (Tempe, AZ; Mesa, AZ; Scottsdale, AZ; Paradise Valley, AZ )
  • ""
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy
  • ""
  • Ability to keep all company sensitive documents secure (if applicable)
  • ""
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service\\
  • ""
  • You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role
  • ""
  • If you need to enter a work site for any reason, you will be required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or similar UnitedHealth Group-approved symptom screener.   • Knowledge of AZ Medicaid & Medicare population

  • Bilingual fluency in English and Spanish
