This role encompasses care coordination for members while they are in the acute inpatient hospital setting and transitioning to the home environment. The discharge care manager coordination entails working with the following roles acute inpatient care managers, medical director team, alternate level of care teams, social workers, behavioral health advocates, facility discharge planners and internal and external member support programs. The discharge care manager role is critical to ensuring the member has a safe discharge to home and to support readmission reduction in accordance with regulatory standards.

Primary Responsibilities:

    ""
  • Coordinate and document the discharge plan in collaboration with other key clinical care team members. Follow the member while in the acute inpatient setting
  • ""
  • Assess patient and family psychosocial needs and develop a transition of care or discharge plan in concert with other members of the interdisciplinary team
  • ""
  • Collaboration with providers and members to coordinate care post discharge
  • ""
  • Participate in rounds with the Medical Director to discuss cases as needed
  • ""
  • Link member/ family to appropriate internal or community-based program support or resources including but not limited to information/ referral to sources of financial assistance, transportation, support groups, and other community services
  • ""
  • Document discharge activities as outlined in standard operating procedures and data entry strategies
  • ""
  • Participate in team meetings, education discussions and related activities
  • ""
  • Works collaboratively with team members in a matrix environment to ensure an end to end positive experience for members, providers, and care teams

Required Qualifications:

  • Licensed Masters degree level clinician in Social Work
  • ""
  • Licenses must be active and unrestricted
  • ""
  • 3+ years of experience in the inpatient acute setting
  • ""
  • 3+ years of experience of discharge planning
  • ""
  • Experience with Medicare, Medicaid, Commercial, and DSNP
  • ""
  • Proficiency in typing skills and software applications that include, but are not limited to, Microsoft Word, Microsoft Excel, Microsoft PowerPoint, and Microsoft Outlook
  • ""
  • Reside in a location that can receive a UnitedHealth Group approved high speed internet connection or can leverage existing high-speed internet service
  • "

  • Experience in managed care

  • Experience using an Electronic Medical Record

  • Problem solving skills; ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action
