The Optum Dual Special Needs Plan (DSNP)at Home program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in their place of residence. The DSNP program combines Optum trained clinicians providing intensive interventions customized to the needs of each individual, in collaboration with the Interdisciplinary Care Team, which includes the Optum clinician, the members Primary Care Provider and other providers, and other professionals. Optum providers serve people in their own homes through annual evaluations, ongoing visits for higher risk members, care coordination during transitions from the hospital or nursing home and ongoing care management.

The Clinical Services Manager is responsible for overseeing Advanced Practice Clinicians and participating in day to day clinical care of patients in the At Home clinical program. This position is responsible for clinical outcomes, business operation targets and overseeing the work activities of team of Advanced Practice Clinicians.

Primary Responsibilities:

  • Set team direction, resolve problems, and provide guidance to members of own team
  • ""
  • Lead, supervise, and develop members of the DSNP clinical team
  • ""
  • Oversee work activities of other supervisors and staff
  • ""
  • Adapt departmental plans and priorities to address business and operational challenges
  • ""
  • Influence and/or provide input to forecasting and planning activities
  • ""
  • Assist in recruiting and hiring clinical staff
  • ""
  • Responsible for market clinical staff performance including recruitment, retention and development of clinical staff
  • ""
  • Drive clinical success around the above program metrics / targets
  • ""
  • Ensure high quality focus embedded into clinical model for DSNP includes Stars and accurate coding and documentation
  • ""
  • Track/monitor trends, gaps, and regularly analyze data related to clinical delivery model
  • ""
  • Assist in strategize, and develop operations in collaboration with the clinical and product teams,to ensure provision of high quality, cost-effective medical services are provided for this membership

Required Qualifications:

    ""
  • Current unrestricted license in state(s) in which practicing
  • ""
  • Certified nurse practitioner through ANCC/AANP for adult, family and/or geriatrics
  • ""
  • DEA License or the ability to obtain
  • ""
  • Basic Cardiac Life Support (BCLS) Certification
  • ""
  • You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role.
  • ""
  • Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained

  • 5+ years of Nurse Practitioner experience plus 2+ years of management or leadership experience
  • ""
  • Managing and developing a clinical team
  • ""
  • Strong business acumen including analysis and business planning experience
  • ""
  • Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
  • ""
  • Prior experience in adult teaching environment
  • ""
  • Proficient computer skills including the ability to document medical information with written and electronic medical records
  • ""
  • Knowledge of Medicare Dual (DSNP) and Medicaid environment
  • ""
  • Proven experience in developing and sustaining internal and external client relationships with healthcare professionals, (DON's, administrators, MDs, RNs, etc.)
  • ""
  • Excellent communication skills and demonstrated ability to foster a culture of clinical excellence and build collaborative relationships
  • ""
