This role will report to the Clinical Program Management and Alignment VP (PMO).

 

This United Clinical Services role will be part of our Patient- Centered Care Model team.   The Patient Centered Care Model aligns population health management programs, improves medical / behavioral / social care integration, and shares analytics with providers; which improve care management. This program leverages marketplace shift from fee for service to shared savings and shared risk arrangements with providers.

Primary Responsibilities:
 
  • Help develop and support the Line of Business ACO VP's (Commercial and Medicaid) in preparing and managing executive communications including things such as:  MBO goals, key performance metrics and indicators, regional and market specific ACO engagement activities, issues, risks, and highlighting best practices for monthly and quarterly business presentations, meetings and report outs.
  • Manage internal LOB team cadence by assisting VP's with development of meeting agendas, notes and acting on follow-up action items.
  • Help develop and implement standard Sr. Executive & Market CEO level reports
  • Manage annual planning and goal setting process with core team of national leaders across Clinical, Network Contracting and Healthcare Economics teams, and market leadership
  • Provides programmatic and tactical implementation support, such as helping set and measure objective success metrics, and tracking team issues, risks and best practice activities.
  • Lead medium to ad hoc projects with development of project charters, scope & operational requirement documents, project plans, identification of resource needs, funding needs, Gantt charts, and status reporting for LOB specific projects and pilots
  • This position makes recommendations to leadership on people, process and technology changes in order to bring project back on schedule and/or within scope
  • Support the LOB needs of the Regional teams to effect improved execution on ACO or MBO level goals with effective management of reported issues, risks, and follow-ups for identification of strategies, actions, pilots or tactics to address needs.
  • Share key LOB activities, strategies, projects and tactic with national PMO team to provide Sr. executive programmatic reporting in the national Gantt, IRAAD and Executive report outs which support Sr. Leadership awareness and communications.
  • Manage work and projects that result in new or enhanced clinical abilities or support models that span multiple functional areas (UHN, HCE, Legal, Clinical Services, Optum, IT)
