Summary: This position is responsible for coordination of all New West patients in all facilities. Primary responsibility is to identify patients that are in a facility and notify the appropriate parties daily. Additional duties include the reconciliation of bed days with internal contacts and the health plans on a monthly basis. This position will also function as a Referral Coordinator in the Medical Management department and be responsible for processing multi-practice requests for all services that require pre-authorization in a manner that ensures accuracy and timeliness.

This is an office-based position. There is some remote option. Full time Monday thru Friday 7 am to 4 pm.

Primary Responsibilities:

  • Primary responsibility is to identify patients that are hospitalized and notify the appropriate parties daily.  The Census Coordinator is also responsible for maintaining database spreadsheets as well as data entry in the referral system to document all admissions.
  • Obtains information through access to the facility admission computer systems.
  • Distributes facility census daily to appropriate teams: providers, hospitalist, Medical Management dept.
  • Notifies the practice sites of any of their patients that are currently hospitalized daily.
  • Receives calls from the facilities throughout the day and updates the census and notifies the Case Managers of new admission
  • Creates and distributes bed day reports.
  • Completes the daily census and enters all appropriate information into the referral system regarding diagnosis, levels of care, discharge needs and discharge date and disposition.
  • Receives, reviews and coordinates all information required to process a referral for pre-authorization for health care services for multiple practices.
  • Demonstrates excellent customer service skills:  warm, respectful, gracious, and courteous in all communications with providers, insurance companies and members.
  • Works collaboratively with Referral Department, office staff of referral providers and contracted insurance companies. Documents communication as required to maintain service excellence.
  • Communicates in a timely manner to referral providers and member regarding referrals that are urgent, denied or that require more information for processing.
  • Maintains the confidentiality of all records and information.
  • Maintains a professional and courteous attitude on the job.  Ensures customer satisfaction and supports the success of the entire healthcare team.
  • Follows all New West Policies and Procedures.
  • Maintains a safe working environment and follows New West Physicians Safety Policies and Procedures.
  • Performs other duties as required or requested in a positive and helpful manner to ensure a smooth-running work area.
