The Behavioral Health Care Manager will operate in conjunction with Genoa Telepsychiatry and Optum Behavioral Care (OBC) to serve as the core member of the virtual collaborative care team for OBC clients. Genoa and OBC are aiming to achieve OBCs vision by building an integrated behavioral health care and primary care collaboration model based on the Psychiatric Collaborative Care Services (CoCM) guidelines from CMS and the Advanced Integrated Mental Health Solutions Center. In this cutting-edge, virtual team, the Behavioral Health Care Manager will interface between patients, PCPs, and psychiatrists to support the mental health and physical health care of patients on an assigned patient caseload from OBC clients. The Behavioral Health Care Manager will have the support of the OBC and Genoa team from the groups clinical and operational teams. Primary Responsibilities:

    ""
  • Use virtual modalities to coordinate care with the patients medical provider and, when appropriate, other mental health providers
  • ""
  • Screen and assess patients for common mental health and substance abuse disorders
  • ""
  • Facilitate patient engagement and follow-up care
  • ""
  • Provide patient education about common mental health and substance abuse disorders and the available treatment options
  • ""
  • Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications
  • ""
  • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment
  • ""
  • Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate
  • ""
  • Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g. cognitive behavioral therapy, problem-solving treatment or behavioral activation) as clinically indicated
  • ""
  • Participate in regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patients medical provider
  • ""
  • Track patient follow up and clinical outcomes using a registry or EHR
  • ""
  • Document encounters in the registry or EHR and use the system to identify and re-engage patients
  • ""
  • Document patient progress and treatment recommendations in EHR and other required systems to be shared with medical providers, psychiatric consultant, and other treating providers
  • ""
  • Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care
  • ""
  • Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment)
  • ""
  • Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseloada

  • 2+ years treating patients with behavioral health conditions

  • 1+ years working in a team-based environment (i.e., with other provider types)

  • Ability/willingness to pursue multiple other state licensed required of the program

  • Dedicated space for home office set up and access to high speed internet services
  • ""
  • Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders.   • Licensed to practice in one or multiple of the target states for the program (AR, PA, MA, MN, VA, IN, TX, CA, FL). These candidates will be highly prioritized.

  • Experience delivering evidence-based therapy
  • ""
  • Experience using behavioral health measures (e.g. PHQ-9, GAD-7, etc.) to track treatment response
  • ""
  • Experience with assessment and treatment planning for common mental health and/or substance use disorders
  • ""
  • Demonstrated experience being detail-oriented, organized, and have exceptional follow-up capabilities
  • ""
  • Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate
  • ""
  • Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., cognitive behavioral therapy, motivational interviewing, problem-solving treatment, behavioral activation)
  • ""
  • Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role

  • Diligent at driving new state license application process with support from the Genoa Telepsychiatry operations team

  • Ability to maintain effective and professional relationships with patient and other members of the care team
