The goal of the Arizona (AZ)  Behavioral Health Advocate (BHA) for Individuals with Developmental Disabilities (IDD) role is to help members live their lives to the fullest by empowering individuals with development disabilities to lead self-directed, healthy, and meaningful lives.  The AZ IDD BHA will coordinate community-based support and services that help enable individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community.  The AZ IDD BHA will additionally coordinate members physical and behavioral health services, Children's Rehabilitative Services (CRS) and limited long term services and supports (LTSS).

Primary Responsibilities:

 

  • Utilize advanced clinical skills to engage members via a member and family centered care planning process
  • Assist members and their families/caregivers with connections to appropriate psychiatric, medical, and psychosocial referrals and services
  • Identify high-risk, co-morbid needs of members who have serious mental illness and developmental disabilities 
  • Identify and remove barriers to procurement, delivery, participation in and success of services
  • Partner with designated external providers, programs, entities to address the needs, gaps in care, and recovery goals of complex and high risk members
  • Partner with parents, family and natural community supports to enable members to receive services while remaining in their homes and communities
  • Partner with internal clinical teams to coordinate a seamless transition for members from an inpatient setting to community based services
  • Assist in the development of a relevant and member-specific aftercare plan
  • Comply with all policies, procedures and documentation standards in appropriate systems, tracking mechanisms and databases
  • Collaborate with providers to determine acuity of mental health concerns, barriers and progress; coordinate appropriate services
