The Physical Therapist is responsible for the evaluation, planning, direction and administration of physical therapy modalities of treatment as prescribed by a licensed physician.  He/she administers treatments as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability.  He/she assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities.

Job Requirements

Required Knowledge, Skills, Licensure, Training & Travel Requirements (if applicable):

• BS in Physical Therapy required. 

• Licensed or eligible for licensure in the State of Oklahoma required.

• BLS required. 

• First Aid certification preferred.

• Crisis Prevention training (CPI) required within 6 months of hire (PRN -N/A)

• Minimum of 1-2 years’ experience inpatient rehab or acute experience preferred. Recent graduates who have applied for the licensure examination may be considered for provisional appointment. 

• Must successfully complete the Pre-employment assessment.

• Basic computer skills required.

 

 

Job Functions

1.

Demonstrates strong working knowledge and skill experience with a variety of diagnostic groups including, but not limited to, the following:  orthopedic and arthritis related conditions; neurological groups including CVA, TBI, degenerative neurological diseases, SCI; generalized weakness as a result of medical or surgical conditions including DM, ESRD, amputation, COPD, CHF, and CABG.

2.

Demonstrates a working knowledge of normal growth and development, related behaviors, needs, comfort, and range of treatment appropriate to the age (adult, geriatric) and diagnosis of patient services to support the provision of an individualized interdisciplinary plan of care.

3.

Demonstrates knowledge of IRF-PAI (Inpatient Rehabilitation Facility-Patient Assessment Instrument) and the Care Tool and their application in payment system for CMS.

4.

Administers evaluation tools according to established protocols; utilizes non-standardized probes for additional assessment as warranted.

5.

Demonstrates the ability to evaluate patient’s and establish an appropriate plan of care, adapting the plan of care to the individualized needs/goals of the patient/caregiver.

6.

Assesses patients pain using hospital approved measurement tools and adjusts treatment          considering behavior, location, severity and frequency.

7.

Implements patient care, with consideration of the interdisciplinary team process and patient plan of care in a knowledgeable and skillful manner, maintaining a high quality of work performance in all areas.

8.

Demonstrates the ability to provide the proper assistance, verbal/tactile cueing et.al necessary for optimal performance and patient outcome.

9.

Effectively treats caseload with ability to appropriately judge required level of assistance patient will need to achieve set goals with gender/age specific criteria in mind and choosing appropriate treatment method, i.e. individual and/or group sessions to best achieve discipline specific treatment goals.

10.

Demonstrates the ability to use time effectively by managing therapy schedule independently and delegating an appropriate portion of treatment to trained support staff as appropriate.

11.

Demonstrates ability to perform home assessment and communicate results to patient/caregivers.

12.

Demonstrates ability to perform/instruct patient in gait training and mobility and use of equipment as needed.

13.

Demonstrates knowledge of management of patients with maladaptive behaviors.

14.

Short and long term goals are established that are comprehensive, realistic, measurable and involve the patient and/or family based on an individualized patient evaluation.

15.

Treatment plan and long/short term goals are established that intersect with the goals of other disciplines when appropriate.

16.

The record is complete, detailed and clearly relates patient level of function in quantifiable terms, treatment methods and strategies, patient response to treatment.  Includes, but not limited to, daily, weekly, team notes, DC summary, education activities and completed according to departmental protocol.

17.

Daily billing completed, including accurate reflection of beginning and ending treatment times and calculation of therapy minutes in the medical record, in compliance with “3 hour rule: with appropriate exceptions documented.

18.

Progress reports are complete with measurable short-term goals and comparing current status vs. initial evaluation status.

19.

Makes recommendations for referrals to other professionals/agencies as indicated based on plan of care.

20.

Patient/Family education is documented in designated and appropriate areas with Home Program provided as needed

21.

Discharge information including home exercise programs for patient/discharge destination is clear and written in "layman's” terms for patient/family to understand.

22.

Appropriate medical terminology and approved abbreviations are used consistently.

23.

Effectively communicates with PTA regarding patient’s performance, changes in medical condition, goal attainment, etc. according to Oklahoma Physical Therapy Practice Act and facility policy.

24.

Therapy boards and patient communication boards will be updated according to departmental protocol.

25.

Demonstrates the ability to effectively review the patient’s chart and communicate/collaborate with multidisciplinary team members regarding patient’s condition.

26.

Communicates with patient/family regarding role of Physical Therapy, expectations of patient, findings of assessment/treatment and seeks input regarding goals of treatment.

27.

Attends weekly team conference and participates in the discussion with the other team members in a constructive manner, supporting collaborative problem solving and interdisciplinary goal setting when needed and identifying progress and barriers.

Assists with training, supervision, and coordination of staff efforts with assistants, students and techs.

32.

Plans, organizes and prioritizes assigned caseload to provide efficient/cost effective care reflective of patient needs based on gender/age specific guidelines and completes caseload within assigned workday according to departmental expectations.

33.

Attends required in-service education, participates and responds to mock drills/emergency situations according to St. Mary’s policy.

34.

Maintains current knowledge of professional practice required for patient population by attending appropriate in-services, unit meetings, continuing education programs and reading related literature and provides in-services to staff on topics pertinent to rehabilitation as requested to contribute to program improvement.

35.

Performs other duties as required and assigned.
