Intensive Outpatient Group Facilitator

Centennial Peaks is seeking a dynamic and talented Intensive Outpatient Group Facilitator.

Centennial Peaks Hospital is a 104 bed hospital located in beautiful Louisville, CO. We are dedicated to providing a full range of mental health and chemical dependency services to the state of Colorado. We strive to enhance the mental health, safety and well-being of our patients, their families, and the community at large by providing ethical, compassionate, medically and psychologically integrated inpatient and intensive outpatient treatment for psychiatric and addiction disorders. The ability to multi-task and thrive in a fast paced environment is critical. 

The IOP Group Facilitator provides quality direct and indirect patient care to individuals in the mental health and chemical dependency outpatient program, Maintains accurate program and patient documentation, including medical records, develops and implements a plan of treatment, in conjunction with members of multi-disciplinary team, conducts groups, communicates with referral sources and other appropriate parties regarding patients' progress, and coordinates patients' discharge planning and implementation. This is a per diem position.

Requirements:

Education:         CD-IOP Facilitator:  Bachelor’s degree in Social Work, Clinical/Counseling Psychology or Social Services related field, as well as additional CACII/CACIII training in substance abuse treatment is required,   Master’s and licensure is preferred.   MH-IOP Facilitator:  Master’s degree in Social Work, Clinical/Counseling Psychology or Social Services related field is required, and licensure is preferred.

Experience:        Prior experience working with a psychiatric mental health population, preferably in an inpatient setting.

Licensure/Certification:         CD-IOP Facilitator:  CACII/CACIII required, LCSW, LPC, LMFT preferred.    MH-IOP Facilitator:  LCSW, LPC, LMFT preferred.

Knowledge:   Prefer knowledge of management, supervision, patient case management, discharge planning, group leadership skills, age specific growth and development, crisis and behavior management, family therapy theory and practice.
